Monitoring



Cyclo interacts with many drugs and these can reduce the efficiency or increase the toxicity of it. Always mention to a doctor or chemist that you are using it. Those drugs contraindicated for use with cyclo include: 

  • Non-steroidal anti-inflammatory drugs (NSAIDs) eg. indomethacin, naproxen. 
  • Antibiotics eg. Cotrimoxasole, trimethoprim, rifampicin, erythromycin, ciprofloxacin, tobramycin, gentamicin. 
  • Anti-epileptics eg. Phenytoin, carbamazepine. 
  • Antifungals eg. Amphotericin B, ketoconazole, itraconazole, fluconazole. 
  • St John's wort (hypericum perforatum).

What monitoring do I need?

It is vital that you receive comprehensive and careful monitoring whilst taking Cyclo. These tests are there to protect you and should indicate a problem before it actually becomes one.

Baseline Monitoring: 

  • History and physical examination 
  • Blood pressure check (BP) ? Complete blood cell and platelet count (FBC) 
  • Blood urea and electrolytes (U&E) 
  • Liver function test (LFT) – though Cyclo does not affect the liver in the same way as MTX. 
  • Creatinine levels (kidney function) – Cyclo can cause renal damage. 
  • Fasting lipid check (cholesterol)

Follow-up Monitoring: 

BP, FBC, U&E, Creatinine levels weekly for first month and then every eight weeks whilst treatment continues.

As Cyclo is a relatively expensive therapy compared to MTX, doctors only prescribe it to those patients who are unable to take MTX or who have had poor results with it i.e. they prefer to offer MTX in the first instance.
