It's because the older avatar picture is still in your browser cache... if that makes sense. Whenever you browse the internet you internet browser will save to your pc all the pictures you view so that should you revisit that page again it loads quicker. As you can appreciate it is quicker to display a picture that is saved to your PC's hard disk than on the internet.
The new image file you are uploading is called sarahc.jpg and the image you are replacing is also called sarahc.jpg which is why your browser gets confused (bless it). As far as your internet browser is concerned it already has the image sarahc.jpg downloaded on you hard drive so it doesn't need to download it again.
If you upload a new picture then delete your internet browsers cache you should see the new picture when you reload the page...
In internet explorer you would need to delete your temporary internet files.. it's under tools, then internet options.
