Stelara Diary - Started 28th Nov 2011
« on: December 05, 2011, 10:22:00 AM »
Hi - thought I'd share my Stelara experience with all.

A bit of background first.

Diagnosed with P in my early 20s. Not really significant and life affecting until my early 30s.

Previous treatments:
**Systemic drugs**:
Acitretin: Nothing positive. Skin on feet completely peeled off. Put on Acitretin straight after Cyclosporin.
PUVA: Cannot take due to additional glasses I will need to wear for UVB protection (current glasses do not have this level of protection and I don't want to wear a visor during work).
Methotrexate: Although prescribed I did not take due to trying for baby #2.
Cyclosporin: No significant results. Major flare up when taken off it. Was stopped abrubtly - should have been weaned off it.
Fumaderm: No response
**Other treatment**:
UVB light
Coal Tar treatments (Short contact dithranol, normal coal tar treatments at various strengths)
Steroids (Hydrocortisone, Betnovate, Dovabet, Elocon, Eumovate)
Dovonex (Scalp solution, ointment and cream)

More recently was on Humira. This didn't work for for - even when my dose frequency was increased to once a fortnight to once a week. My Humira diary is pasted in Pie's "Humira Here I Come" thread. (Page 119)

Stopped Humira on 19th September 2011. Whilst in wash-out I had UVB light treatment between 11th October to 18th November 2011.

and here the diary commences ;)

02Nov11: Appt with derm. Go ahead given for Stelara
21Nov11: Stelara delivered
28Nov11: First injection provided @ 1830
29Nov11-01Dec11: Very minor headache
02Dec11: Cough starting (Wife and daughter have had a cough/cold for last week though and daughter sneezed in my face this morning! So not sure whether this is related to Stelara). No significant improvement yet.
03Dec11-04Dec11: Minor sore throat
05Dec11: Feel a bit achey. Sore back and tight chest. Where improvement occured with UVB I can see P spots over these areas but not quite as scaley as pre-UVB - but not as good as when on UVB :(

06Dec11: Back muscles ache a little - but could be gym and the fact that during the day I've been stuck at my desk for hours at end. But could equally be the Stelara. Legs particularly more flakey than normal and facial P not receding at all. So it's been 8 days now since my first injection and I've not seen any significant improvement.  :(
Read a depressing comment on another board recently (ironically the "Inspire" forum!) where someone mentioned that Stelara was unlikely to work for people that didn't see any significant improvement on Stelara within the first 4 days of the first injection   :-\

Keen to get other people's experiences on when Stelara first kicked in for them.

Hey Pie - hope all is well pal!
Thanks for that info - I did see those figures on a number of sites and it was also on the Stelara leaflet given to me before I started.
What I'm keen to know is when did people actually see any significant improvement. ie. Was it after a couple of days or weeks. Read through HK's Stelara diary and aware Stelara didn't immediately work it's wonder with her. There definitely seems to be a lack of Stelara experience on this forum - hopefully that'll change soon.
Must be great for you going into another Christmas P-free - let's hope these continue forever!

Hey there Harvin, what's the latest? Any progress?

I started on Stelara at about the same time as you did, just a little bit later, on 8 December. Still no change for me.

Will be interesting to see how it goes for both of us.

Please let us know how you're doing.

And happy Christmas!  :)

Hi there - been meaning to post but last few weeks (months even!) have been crazy at work.
Anyway - here's what's been happening over last couple of weeks.
Not a drastic improvement that other people may report but I do feel it is improving slowly...

11th Dec: Improvement around face. Some resistant patches near eye and eyebrow have disappeared but near scalp line and behind ears still stubbornly remaining.
18th Dec: Not much change from above. Legs definitely less scaley and noticed improvements on hands and feet. Nails still pretty bad.
19-22nd Dec: Arms getting a little worse but I put that down to 3 Christmas parties, too much beer and not enough water. However, overall it's still pretty good. Stubborn areas include small patches behind ears, patches below elbox and bottom part of back. Nails flared up - not sure if I've just been unlucky and had infection above nails but strange thing was they all flared at same time. Very painful above cuticles - but sure this is not down to Stelara.
23rd Dec: Improvement all on cuticles (swelling down) except right hand little finger. Bottom of back and top sides of legs on bum not improving. Left arm still a little itchey and not much improvement since I stopped UVB. However, with all that said Stelara is making an overall small improvement. It's difficult as UVB is a short term way to improve skin and I started Stelara after my skin had improved quite a bit from UVB. 2nd injection next Friday and I'm fairly confident I'm going to see more improvement.

Wrt arranging for nurses to come round to inject I'm having a problem getting this organised. When the first injection was arranged a nurse mentioned she would be round on Boxing Day evening. After an initial 4 chasers I've now got a date of 30th Dec - but no time. I've chased a further 3 times to get a time after being told a nurse will ring me on a particular day to arrange and not hearing anything. Rather annoying as the previous nurse told me I should've been ready with the injection 30mins out of fridge when she arrived (she was obviously in a big rush). I felt like explaining I risked having the Stelara sitting in a warm room for over 30mins whilst I waited for a nurse who was obviously running behind schedule - didn't bother as I'm needle phobic and didn't want her taking it out on me with a nasty jab :)

Worth taking some photos to benchmark any change. It's difficult to notice any improvement as you live in your skin day on day. My wife says she notices improvement - so might be worth you getting someone else to opine on any change in your skin.
Hopefully it will kick in soon for you. However, with Christmas coming up and all alchohol and over eating that normally comes at this time of year you may not see any significant improvement until the New Year - but fingers crossed anyway!

I'll try and post a bit more and interested in hearing how you get along too. Having tried and failing with Humira I'm really hoping this does the trick.

Merry Christmas!

04Jan12: P spots appearing over body - itchy. Overall much improvement.
23Jan12: been calm for last few weeks. Legs now itchy. Arms not improving. Couple of small P spots appeared on tummy.  Still great improvement.
27Jan12: more pustular P spots appearing over body and legs now having a few plaques re-appear. Improvement appears to be slowing down - possibly on the cusp of failing to improve further. Time will tell.

Hi Pie. Haven't taken pics for a while.
However, the Stelara had made a great improvement. Face all clear. Legs cleared, but left with sme patches on arm and one bit on my ankle. Strangely all these turned pustular.
About 4-5 days ago the pustular P dies down and my legs and arms started really itching again and my plaques have been gradually appearing again.
It's as if my body needs another Stelara jab as it's been a month since my last jab. Unfortunately as I had my week 0 jab at end of Nov and my week 4 jab at end of Dec I now have to wait until end of March for my 3rd jab :(
Left a call my Derm's secretary to advise me what my options are but as he's only got limited experience with Stelara I'm not feeling terribly optimistic.
Had a search on the net and it appears I'm not the only person to get a relapse after the 2nd jab and into the 3rd month.

How are all you other Stelara users doing? Keen to hear from you all!
