Haven't had to sign a form, but I have been asked how many treatments of UVB I have had, I think they say that one sessions a year for three years is safe, then after that it depends on how bad you are and probably cost, especially if, like me, it is not that beneficial.

Clear for 7 months.Starting to re-appear on legs and back but controllable with Alphosyl HC.

I think it only started to come back because of the very stressful period I have been going thru.
Spoke to Derm last week and she reckons that I should give it a few more weeks to see if it clears up before considering further UVB
